Automatic Payment Plan for Vehicle and Home Equity Loans - If you'd like more convenience and easier record-keeping, give our automatic payment plan a try.


Direct Deposit Authorization Agreement - Provide your employer with the information they need to deposit your pay directly into your account.

Preauthorized Transfer Request - Transfer funds between State Farm Bank and another bank. Funds in your checking, savings, money market, health savings, or IRA money market accounts are eligible for transfer.

  • Traditional IRA Contribution Instructions - Deposit funds into a traditional IRA. One-year CDs, CDs at maturity, and IRA Money Market Saving accounts are eligible.
  • Roth IRA Contribution Instructions - Deposit funds into a Roth IRA. One-year CDs, CDs at maturity, and IRA Money Market Saving accounts are eligible.
  • Prior Year Deposit Form - If you haven't already contributed the maximum amount allowed by law, you may be able to contribute towards your Traditional IRA, Roth IRA, or Health Savings Account (HSA) for the prior tax year. Review the form for applicable rules and regulations.
  • IRA Distribution Form - Request a one-time or recurring distribution for your Traditional IRA or Roth IRA.
  • IRA Transfer / Rollover Form - Transfer funds from an IRA, 403(B), Qualified Retirement Plan (QRP), or Governmental 457 Plan to a State Farm Bank IRA.
  • Traditional IRA Designation or Change of Beneficiary Form - Choose who will receive your traditional IRA funds should you pass away. A new form will replace any previous designations. Please list any previous names on the new form that you would still like to keep as your beneficiary.
  • Roth IRA Designation or Change of Beneficiary Form - Choose who will receive your Roth IRA funds should you pass away. A new form will replace any previous designations. Please list any previous names on the new form that you would still like to keep as your beneficiary.
  • Preauthorized Transfer Request - Transfer funds between State Farm Bank and another bank. Funds in your checking, savings, money market, health savings, or IRA money market accounts are eligible for transfer.
  • Multiline-line IRA Allocation - This form is used to split tax qualified rollover funds between State Farm Bank, State Farm Life Annuity and/or State Farm Mutual Funds.

  • Mistaken Distribution Form - Use this form if a mistaken distribution occurs and you'd like to repay that amount to your HSA.
  • HSA Rollover / Transfer Request - Use this form to transfer funds from one Medical Savings Account (MSA), Health Savings Account (HSA), Flexible Spending Account (FSA), Health Reimbursement Arrangement (HRA), or Individual Retirement Arrangement (IRA) (not including ongoing SEP and SIMPLE IRAs), to another. Funds can be moved from an external bank to State Farm Bank, or from one State Farm Bank account to another.
  • HSA Designation or Change of Beneficiary - Choose who will receive your Health Savings Account (HSA) funds should you pass away. A new form will replace any previous designations. Please list any previous names on the new form that you would still like to keep as your beneficiary.
  • Preauthorized Transfer Request - Transfer funds between State Farm Bank and another bank. Funds in your checking, savings, money market, health savings, or IRA money market accounts are eligible for transfer.
  • HSA Distribution Form - Use to request a distribution or a removal of an excess contribution.


Coverdell Education Savings Accounts are tax-qualified. After completing a tax-qualified account form, fax it, along with a cover letter, to 855-261-6713. Or mail it to State Farm Bank Operations, Tax-Qualified Unit, P.O. Box 2316, Bloomington, IL 61702-2316.
  • CESA Contribution Instructions - Deposit funds into your Education Savings account (eligible CESA accounts are one year CD's and CD's at maturity).
  • CESA Designation or Change of Beneficiary - Choose who will receive your CESA should you pass away. A new form will replace any previous designations. Please list any previous names on the new form that you would still like to keep as your beneficiary.
  • CESA Distribution - This form is used to request a one time or recurring distribution for your CESA.
